Member engagement

Reach members, and track whether the action actually got done.

A reminder goes out, a call is placed, a portal nudge is sent. Most engagement is measured there, at the point the message leaves. We reach members across the channels they use and follow the action through to whether the screening was scheduled, the gap was closed, or the form was completed.

The problem

Sending the message is only the first step.

Member engagement runs on reminders, calls, and portal nudges, usually tracked by how many went out. What is harder to see is what happened next: whether the screening was scheduled, the care gap was closed, or the assessment was completed.

Outreach is also spread across teams and channels that are not connected, so it is difficult to know which members still have an open action, or which message and channel got people to act. The harder problem is reaching the right members and getting the action completed.

How it works

From the member who needs to act to the completed action.

01
Who
Start from the members who need to act, surfaced from population and risk data.
02
What
The specific action for this member: a screening, a care gap, a form.
03
Reach
Across voice, text, and digital, on the channel the member uses.
04
Done
Confirm whether the action was completed, and follow up when it was not.

Following the action through to completion is what makes it possible to see which members acted, and which outreach actually worked.

What we do

Member engagement capabilities

Target the right members

Start from the members and actions surfaced in population and risk data, so outreach goes to the people who need to act rather than a blanket list.

Match the action to the member

Pair each member with the specific action that applies to them now, such as a screening to schedule, a care gap to close, or an assessment to complete.

Reach across voice, text, and digital

Reach members on the channels they actually use, coordinated so a member gets a consistent message rather than the same ask three different ways.

Make the action easy to take

Let members schedule, confirm, or complete the action in the same interaction, so outreach leads to a completed action rather than another to-do.

Confirm completion and follow up

Track whether the action was completed and follow up when it was not, so an open care gap does not quietly stay open.

Bring in a person when needed

Automate routine outreach and follow-up, and bring in your staff when a member needs a person. Member consent and experience are respected at every step.

What we measure

The measure is how many actions got completed.

For each engagement, we follow the path from outreach to the action that mattered: a screening scheduled, a care gap closed, an assessment completed, or a member handed off to the right team.

Let’s look at how your outreach turns into completed actions.

Show us how your team reaches members today and how you know whether the action got done. We will identify where outreach can be better targeted and where more of it can end in a completed action.
